Intake Procedure: Families are encouraged to visit/tour the program and meet with the Associate Director or designee. Families choosing to use our services are provided with the necessary paperwork. Assistance in completing the paperwork is available if needed.
Intake Requirements: Completed paperwork needs to be submitted prior to the first day of services with the exception of the Health Assessment which needs to be submitted within the first 60 days of enrollment.
