Assessments completed for individuals applying for Agency services.
- Home Delivered Meals, Personal Care, Personal Emergency Response Systems, Adult Daycare and the Caregiver Support program.
- Care Management is provided by trained Aging Care Managers.
- Level of Care Assessments provided to individuals applying for waiver programs or facility placement.

Eligibility: Most programs are provided to those who are 60 years of age or older. Assessments completed for individuals applying for waiver programs or facility placement.

Consumers may be eligible for Cost Share or an income-based fee scale.
Other payment options may include services provided on donation basis (or no fee), payment through an outside agency (VA or MH/ID/EI), or private pay.
Wait List: May be placed on a wait list depending on program capacity and how many are interested in program.
