Intake Requirements: Patient is required to sign consent for services and notice of privacy policy forms. Patient must also complete an intake form.
Intake Procedure: Upon scheduling appointment patients are texted a consent for services, notice of privacy policy, and intake form. Patient must complete forms either online or at their office before services given at first appointment. Staff will confirm information at time of visit.
