Intake Requirements: Need for care in your home. Eligibility requirements for various services are different, call to talk to an intake specialist to see what services are available to you. Patients must live in our service area.
Intake Procedure: Referrals can be self-made or made by physicians, hospitals, clinics, IV companies, or insurance companies by calling Central Intake at 724-282-6806 Option 4
Helpful Tips: All consumers have the right to choose the home care that best suits their needs. You cannot be forced to take an agency you do not choose but you may need to ask for the agency you prefer.
Fee Amounts: Most home health and hospice services are covered by personal insurance. Private duty, Emergency response systems and medication dispensers are primarily self-pay.
