Intake Procedure: Call, walk-in, visit website
Intake Requirements: Intake staff will provide information on the documents needed to begin the application process. Examples of documents that may be needed are: Medicare card, other insurance cards, documents to verify financial eligibility, photo ID, power of attorney documents, etc.
Fee Amounts: Financial eligibility will be discussed with Intake staff.
