Eligibility:
- U.S. citizen
- PA resident at time of injury and application
- traumatic brain injury after July 2, 1985
- have exhausted all resources for head injury care
- be 21 years of age or older
- meet income guidelines
- complete an application
- be assessed to determine that needs can be addressed by the Head Injury Program through rehabilitation.

Intake Procedure:
To enroll call 717-772-2762 or 1-866-412-4755. After answering a
few simple questions, the Enrollment Specialist will mail a Head
Injury Program application to you. You can also expect to be
contacted by a Pre-enrollment Assistance Coordinator who will offer
you individualized support in completing the application as well as
assist in identifying any other statewide resources.
