North Western PA Optical Clinic - Low-Cost Prescription Eyeglasses
Provides new prescription eyeglasses, including lined bi-focals, for $40 to low-and moderate-income individuals and families who meet the eligibility requirements. Transition lenses, no-line bifocals and solid tint lenses are available for an addtional cost. Individuals must present their own prescription from their doctor and will have the opportunity to chose from lenses available.

1 - $21,660
2 - $27,992
3 - add an additional $6,960 for each addtional household member.
Hours: Monday, Tuesday, Thursday, 9:00am to 2:00pm; Wednesday, 9:00am to noon.

Intake Requirements: Clients must have a doctor's prescription for the lenses.
Intake Procedure: #1 Meet the income guidelines. Income guidelines, based on household size and maximum gross income are as follows:
1 - $21,660
2 - $27,992
3 - add an additional $6,960 for each addtional household member.
#2 Applicants must present an eyeglass prescription from an eye doctor.
#3 Make an appointment at the location conveinent to you. Appointments are required; no walk-ins accepted.
#4 Applicants will chose from frames available and new eyeglasses will be sent via mail directly to applicant's home. Glasses will arrive in approximately two weeks.
#5 Payment is required at the time of appointment. Cash or money orders accepted; no personal check.
Fee Amounts: $40
Facility/ADA access: Building entrance, restrooms, and within the building.
