The following is a brief explanation.
  • Lacks a fixed, regular, and adequate nighttime residence
    • Primary nighttime residence is a place not ordinarily used for sleeping ie. car, park, abandoned building, etc
    • Living in a supervised, temporary shelter
    • Will lose permanent housing within 14 days or less
    • Is an unaccompanied youth or in a family experiencing homelessness
    • Is fleeing or attempting to flee a domestic violence condition
  • Hours: See local office hours. Hours vary but most sites are open between 8:30am and 4:00pm. Check as not all sites are open Monday through Friday.

    Intake Requirements: For an initial (first time) request, you must visit a local Driver Licence Center and let the counter staff know. Must bring one acceptable proof of identification:
  • Birth Certificate
    • Passport
    • Social Security Card or a Social Security Administration letter of ineligibility

      Proof of mailing address:
    • W-2 Form
    • Utility Bill (phone bill)
    • Letter from the shelter to verify mailing address
