Mental Health Housing Programs
Only referrals through HMIS by a Housing Specialist will be considered

Projects for Assistance in Transition from Homelessness (PATH) provides rental assistance to adults who:
* Have a documented serious mental illness or dual diagnosis (SMI and Substance Use)
* Receive sufficient income to maintain housing after receiving assistance

New residence - Security deposit for people who have no permanent residence, have a documented serious mental illness and receive sufficient income to maintain housing after receiving assistance.

Eviction - Assistance with a one-time back rent payment if no more than one month rent is owed.

Eligibility: Diagnosed with serious mental illness and can provide documentation. Must have income that proves that the individual can sustain the desired living situation for at least 90 days. Is currently homeless or at imminent risk of homelessness.

Intake Procedure: Individual/families must have a referral sent by a housing specialist to receive these services. Once a referral is received, a call, within (3) business working days, will be placed directly to the individual to follow-up the current information and determine if the individual meets the eligibility for services.

At that point, an intake appointment is scheduled with the individual. Individuals will need proof of income for the household, documentation of serious mental illness, amounts of monthly bills, and eviction notice or lease if seeking security deposit assistance.
Fee Amounts: Services are provided at no cost.
Wait List: Depends on the program.
Facility/ADA access: wheelchair accessible
Languages Spoken: English, Interpreters provided based on availability
Capacity Limitations: Assistance available until funds are depleted.
