Provides services and supports to assist youth and young adults with a mental health diagnosis overcome barriers to independent living.

Eligibility: Residents of Bucks County, ages 16 to 26, enrolled in Medical Assistance (Medicaid), who have had a psychological or psychiatric evaluation completed in the last 12 months
Hours: Office Hours: Monday through Friday, 9:00am to 5:00pm
