The Primary Health Network is a Federally Qualified Health Center (FQHC) that provides Primary Care, Dental, Behavioral Health, and Specialty Services. Offices are located in 11 counties in Pennsylvania and one in Eastern Ohio. Primary Health Network accepts all patients, regardless of their insurance status, and offers a Sliding Fee Scale to eligible patients. The Sliding Fee discount qualifications are based on household income and size. Assistance with the Health Insurance Marketplace to the community members and patients is also available.
