Intake Requirements: For SelectPlan: one month worth of pay stubs,social security card, birth certificate, and photo ID.
Intake Procedure: Income eligibility to see if eligible for SelectPlan for Women. Private insurance welcome. Check to see if they meet state criteria (U.S. citizen, age 18-64, resident of PA, and contracepting).
Fee Amounts: Eligibility for free or partly free services for age 18 and over based on poverty guidelines; Sliding fee scale; under 18 free

  • Sexually transmitted disease (STD) screening and treatment

  • Breast/cervical cancer screening

  • Community education and outreach

  • Educational materials, films and speakers for schools, churches, parent groups and community agencies
