Eligibility: - Must be a VETERAN OR VETERAN FAMILY WITH THE VETERAN AS THE HEAD OF HOUSEHOLD; excluding those who have a dishonorable discharge

- Very low-income: Gross household income must not exceed 50% of the median income guidelines

- Must be homeless or at imminent risk of becoming homeless (must have a formal eviction notice)

- Must have at least one day active duty to qualify for SSVF funding

Intake Requirements: Photo ID, copy of DD-214 or VA medical card, proof of income, social security cards and birth certificates for all family members to be served, any lease/rental statement, past due notices, utility bills
Intake Procedure: Call to schedule appointment for a full assessment and to obtain an application. Call if you are unsure if you will fully qualify. There are other programs they may be able to share.
