Hours: Office: Monday, 8:00am to 8:00pm; Tuesday through Friday, 8:00am to 5:00pm; Closed 12:00pm to 1:00pm

Intake Procedure: Call the access center to start traditional outpatient services and be placed on a waiting list; Call CADD directly to receive information and criteria for this specialty care
