Eligibility: Single adult household diagnosed with a documented serious mental illness or dual diagnosis (SMI and Substance Use) who is currently literally homeless (HUD Definition 1), meets criteria for chronic homelessness, and can provide required documentation.

Intake Procedure: Contact 211 or the Franklin County Access Center to complete an assessment. Once completed the Housing Program Specialist, will be notified if an individual may be eligible for the program. The Housing Program Specialist will contact individuals to make an appointment for an intake and discuss documentation that will be required to determine eligibility.
