Provides replacement record for individuals who received their COVID-19 vaccine at one of the Allegheny County Health Department (ACHD) Vaccine Clinics.

Eligibility: ONLY individuals who received their vaccine through ACHD clinics can obtain replacement records through ACHD.

For all other sites, people should contact the original vaccine provider or use the Statewide Immunization Information System (SIIS) to get proof of vaccination. Call 2-1-1 for assistance.
