Provides free medical and dental care to uninsured individuals whose total household income is no greater than 250% of the federal poverty level. Medical services include routine medical visits, chronic disease management, health screenings, medication therapy management. Specialty care includes cardiology, endocrinology, gynecology, ophthalmology, physical therapy, podiatry, urology, gastro-intestinal, ear, nose and throat, nutrition counseling, orthopedics and rheumatology.

Eligibility: Between the ages of 18-65 and do not qualify for Medicaid or
other government programs.
Total household income does not exceed 250% of the Federal
Poverty Guidelines (FPG).
Employer does not provide medical or dental insurance; or cannot afford insurance premiums offered through the private or public marketplace.
Hours: Weekdays, 9:00am to 4:00pm
Appointment required.
