Disability Options Network - Don Services - Homemaker Services
Enables individuals or families with whom the individual resides to maintain their private residence. This service
can only be provided when a household member is temporarily absent or unable to manage the home, or when no landlord or provider agency staff is responsible to perform the homemaker activities. Homemaker Services must be provided by
a qualified homemaker and may include cleaning and laundry, meal preparation, and other general household care.

Eligibility: People of any age with Intellectual / Developmental Disabilties on PA's Consolidated or P/FDS Waivers.

Intake Requirements: Ask for LiNKS to Everyday Life Program. PA's Consolidatedor P/FDS Waiver. Medicaid number, birthdate, social security number and other demographic and functional information will be taken at the initial assessment. Potential participants will need to authorize their Supports Coordinator to share information with our program. Before services can be authorized, an ISP meeting will need to be held.
