Northern Area Multi-Service Center
NAMS - Caregiver Support Project
AARP, Southwest PA 2-1-1 and NAMSC are partnering to provide a free Caregiver Support Program to unpaid (informal) family caregivers who provide care to adults 18+.
The Caregiver Support program links caregivers like you to a "caregiving coach" who is an expert in senior services. The caregiving coach would contact you to connect you with available caregiving resources and offer ways to reduce caregiver stress.
During the program, we hope to learn more about caregiving: what the balance of care for your loved one includes, what your primary concerns are, what types of services or resources you're seeking and any opportunities or gaps in resources that exist for you.
In addition to reducing caregiver stress by creating awareness of available caregiving resources, we seek to use the information to grow our statewide 2-1-1 database to be more responsive to caregiver needs.

CareStar - Community to Home Program
Provide assessment and care planning over a 6-month period. Community Health Workers can assist families in attendance to appointments, roadmaps to self-sufficiency, linkage with community resources, and support and understanding as they navigate their child's diagnosis.

Newly diagnosed children with special health care needs or newly residing in a PA rural county.
Households with an income of 300% or less of the federal poverty level.

Delaware County Office of Services for the Aging - Caregiver Support Program
The program provides the primary caregiver with reimbursement for out-of-pocket costs associated with caregiving such as respite, caregiving-related services and supplies. The program offers education, training, counseling and more. Each caregiver is assigned a care manager who will come to the care receivers' home, conduct a comprehensive assessment of the needs of the caregiver, provide support, and work with them to develop a person-centered plan of care.

Offers well-trained and experienced Family Support Partners with personal experience raising a child with behavioral issues who can help with connections to community services, negotiating different systems, or emotional support. The families can also call to request a face-to-face one-on-one Family Support Partner through the Parent Support Line.
