The Family Caregiver Support Program assists in reducing stress for primary caregivers with functionally dependent persons age 60 and older, or under 60 years of age with a diagnosis of dementia. Assessment, care management, information counseling, benefits counseling and caregiver training are free of charge. Financial support may be provided for purchase of supplies and services, home modifications, assistive devices and respite care.The Intergenerational Family Caregiver Support Program is a reimbursement program for a grandparent, step-grandparent of a child, or a relative of a child by blood or marriage who is: 60 years of age or older and lives with the child, the child's primary caregiver because the biological or adoptive parents are unable to serve as the primary caregiver, has a legal custody or guardianship or is raising the child informally, and the child is 18 years of age or younger. Family Group Decision-Making allows family members as defined by the consumer the option of becoming involved with the consumer's care needs, and to assist them in developing strong supports that meet their needs. This program is an opportunity to allow our consumers and family members to be empowered to make the decisions and changes necessary to keep our consumers safe and ensure their needs are being met.

This program is offered to all parents of newborns in York County, with priority given to first-time parents. A coach is provided free at a convenient time in your own home. Visits are scheduled during your new baby's first few months home from the hospital. Each enrolled parent receives a free gift, home visits by a coach and, if desired, free classes on parenting infants.

The support line is also available to respond to inquiries about the caregiver benefits associated with the Caregivers and Veterans Omnibus Health Services Act of 2010.
