- Heart Assess: designed to improve quality of life for heart failure patients. Patients are assigned home health care nurses to help with medications, diet, disease management and assisting with lifestyle changes.

- Breathe Easy: allows patients who suffer from chronic obstructive pulmonary disease (COPD) to better manage their condition at home.

- Tele-monitoring: remotely monitors vital information, such as blood pressure, heart rate, and blood oxygen levels for cardiac patients or any patients who need additional supervision; patients are also reminded when it's time to take their vital signs, then guided through the process with a gentle, friendly voice. Results are automatically transmitted to a Holy Redeemer nurses.
