Roads to Freedom Home Care Service enables adults with disabilities, who are nursing facility eligible, to remain in their own homes and continue to live with support and services. Services offered include assistance with bathing, dressing, transferring, medication reminders, shopping, transportation, or other activities specific to the individual's plan of care. Roads to Freedom Home Care accepts Community Health Choices, OBRA waiver, Act 150 program, and private pay.

The Adult Autism Waiver (AAW) is a 1915(c) Home and Community-Based Services (HCBS) Medicaid waiver designed to provide long-term services and supports for community living, tailored to the specific needs of adults aged 21 or older with ASD. The program is designed to help adults with autism spectrum disorder participate in their communities in the way they want to, based on their identified needs.

Develops and implements creative alternatives to placing Medicaid-eligible individuals in medical facilities such as nursing homes and Community Residential Facilities (CRF). The Aging Waiver Program offers eligible participants an alternative to institutional care.
