Community Interactions - Home Based Waiver
Provides support for individuals who live at home. The Aging Waiver Program offers eligible participants an alternative to institutional care. Home Care
Roads to Freedom Home Care Service enables adults with disabilities, who are nursing facility eligible, to remain in their own homes and continue to live with support and services. Services offered include assistance with bathing, dressing, transferring, medication reminders, shopping, transportation, or other activities specific to the individual's plan of care. Roads to Freedom Home Care accepts Community Health Choices, OBRA waiver, Act 150 program, and private pay.

Eligibility: Roads To Freedom does not assess eligibility for consumers to enter the program. Consumer’s are previously assessed and deemed eligible through the Independent Enrollment Broker (IEB) and their local county assistance office. If a consumer is found to be nursing facility eligible along with financially eligible they will be placed into the Community Health Choices program and able to receive services through Roads To Freedom. If a consumer is nursing facility eligible but over the income guidelines they may be placed in the ACT 150 program which is fee-based. Roads To Freedom cannot determine a person’s eligibility to receive services through Community Health Choices, ACT 150, or OBRA.

The Adult Autism Waiver (AAW) is a 1915(c) Home and Community-Based Services (HCBS) Medicaid waiver designed to provide long-term services and supports for community living, tailored to the specific needs of adults aged 21 or older with ASD. The program is designed to help adults with autism spectrum disorder participate in their communities in the way they want to, based on their identified needs.
