Information and referr

Helps consumers live in their homes and communities with dignity and continued independence. Offers day services and recreational therapy, laboratory and diagnostic services, vision, dental, hearing and podiatry care, prescription drugs and medication management, medical supplies and equipment, physical, occupational and speech therapy, social services and behavioral health care, home health care and personal care, in-home nursing care, acute hospitalization and ambulance services, 24/7 access to emergency/urgent care, nutritional counseling and meals, transportation, dental services, and chaplaincy services.

Provision of non-medical, in-home personal care that enables people with disabilities including veterans and the elderly to live in their own home community rather than in an institution. Services provided include:
- Personal assistance
- Bathing/dressing assistance
- Meal planning/preparation
- Laundry/light housekeeping
- Medication reminders
- Shopping/errands
- Night supervision
- Respite for caregiver

Provides coordination of services to people with physical disabilities and seniors through Community HealthChoices. Assists in developing plans that may include the following services that enable individuals to live safely and independently at home: personal care attendants, durable medical equipment, home delivered meals, home modifications and supportive employment.

Serves a 3-course meal. Also offers a choice produce market with seasonal fresh options weekly.

Provides a free take out community dinner (pickup in rear parking lot).

Offers summer feeding Monday through Thursdays at all elementary schools, plus participating middle schools and high schools, and mobile sites at Jamestown and Oakbrook. Serves breakfast and lunch. Meals are not available on weekends.
Program starts June 20, 2023 and ends July 13, 2023. RKAA City Line continues till August 30, 2023. Oakbrook at Berks Community Pavillion continues to serve till July 27, 2023.

UCHC Meal Service offers hot meals five nights a week and sandwiches on Saturday for poor and homeless persons in West Philadelphia. Meals are given at different locations throughout the week.

Chandler Hall opened its Apartment Style Residences in early 2000. There are 4 lovely Manor Houses comprised of 24 apartments in each house. The common areas of each house provide tastefully appointed living spaces; these spaces include living, family and intimate dining rooms. Residents enjoy menu selections for three delicious meals per day. This personal care setting allows for many resident needs to be met as well as a variety of programming. Residents enjoy a smaller community within the benefits of a larger community.
