    Project HOPE improves the lives of those living with HIV/AIDS and those at-risk for the disease by providing medical case management services, including supports for food, transportation, nutrition, psychosocial, and housing (HOPWA) for those that qualify.

    Our HIV Navigation Services, a short-term case management program, continues for residents at high-risk of contracting HIV.

    In addition, Project HOPE provides HIV and STD testing, prevention, education, and outreach throughout Montgomery County.

    Offers free anonymous and confidential HIV testing in our facilities and conducts outreach testing in the community. Trained counselors and testers use a simple and quick test that involves a blood draw or a finger stick (blood) collection rapid testing.

    How do I get my test results?
    Regular Testing, after your blood, is drawn and submitted to a lab, testing normally takes two weeks for the results to return. To get the results, simply present your proper identification. No results will be given without proper identification.

    Rapid Testing, test results are given 15-20 min (pending on testing method used) after the sample is collected. Rapid testing cannot be performed anonymously. All testing is done in conjunction with counseling. Results will not be given via the telephone

    - Investigation and control of reportable diseases. Educates patient regarding possible source, transmission, and prevention/control measures. Investigations include reportable communicable and non-communicable diseases and food and/or waterborne disease outbreaks
    - Outbreak investigation of multiple cases of illness linked to a common source. Gathers information regarding symptoms, onset, activities, food and beverage history of ill versus non-ill persons. If threat of an outbreak that mass immunizations may prevent or limit, the center may quickly establish mass immunization sites among the non-immunized population.
    - High risk infant follow-up program for families without health insurance, who have premature infants, or infants with congenital defects. Home assessment, health education, and growth and development monitoring are provided by a nurse.
    - Information and referral services on Hepatitis C, AIDS, genetic disorders, cardiac, cleft palate, cystic fibrosis, phenylketonuria, spina bifida, sexually transmitted diseases. If there is a program at the state level for specific disorders, the staff will assist local residents in accessing these services.
    - PKU follow-up provided to infants with elevated phenylalanine levels. Nurse will act as a liaison between the family and the PKU Treatment Center.
    - Rabies prevention. All animal bites are reported to the State Health Center. Both victim and owner are contacted and informed of protocol for confinement of the animal. If there is a risk of rabies, a nurse will assess and educate the parties involved regarding the need for rabies prophylaxis.
    - Community Education Programs on such topics as: AIDS, diabetes, health risk education, high blood pressure, and nutrition. Farm and fire safety education programs for health fairs and parochial schools, including Amish schools. Health and safety educational programs for day care and HeadStart staffs and WIC parents.
    - Provides information on West Nile Virus.
