Information and tools available can help individuals and their support network to better understand chronic pain and work more effectively with individual health care teams toward a higher quality of life.

Palliative Care is available to participants who do not desire to seek aggressive medical treatment and are interested in managing pain and other symptoms. A person may still be seeking treatments and/or therapies while under palliative care. Care is generally provided in a person's own home; however, palliative care is also available in a hospital, nursing home or other medical and/or residential setting.

Provides in-home medical care services, including skilled nursing, wound care, IV/antibiotic therapy, medication education, chronic disease management, diabetes management, pain Management, catheter care, physical therapy, occupational therapy, speech therapy, and medical social work.

Healthcare and Rehabilitation Center that provides quality care in a home-like environment. Short-Stay and Stroke rehabilitation. Provides therapy, health management, and amentities like social activities.

Provides individualized post-hospital skilled nursing care in a comfortable environment. Clinical and therapy teams are experienced in providing specialized care focused on individual needs, interests, and ability. Specialty Services may include but not limited to: physical/occupational/speech therapy, Onsite Radiology, Dialysis Care, hospice care, and pain management and Transportation Services. Amenities and accomodations may be offered.

Offers physical, occupational, and speech therapies as well as more specialized outpatient services. Provides physical, occupational, and speech therapy offered seven days a week. Treats a variety of diagnoses including orthopedics, strokes, cardiac, wounds, and lymphedema.

To help individuals manage their pain and end their dependency on narcotic medication.

Performs hip and knee replacements and all other types of orthopedic surgeries. Provides acute rehabilitation, transitional care, and outpatient physical, occupational, and speech therapies.

Provides comprehensive treatment for substance use and the associated physical, social, and psychological disorders. Services include hospital-based and residential detoxification, short term rehabilitation, and moderate term rehabilitation.
