Lehigh Valley Assertive Community Treatment
Multidisciplinary community treatment teams provide intensive, individual support to individuals diagnosed with a serious mental illness in both Lehigh and Northampton counties. MHICM advocates a philosophy of recovery and rehabilitation. Veterans enrolled in this program have an assigned intensive case manager who has frequent contact with them through regular visits. The case manager will facilitate their integration into the community using natural support systems, such as family, landlords and employers, whenever possible.

Our integrated, multidisciplinary team delivers services that provides intensive, flexible community support, improves health status by reducing psychiatric symptoms, and decreasing inpatient hospital use, focuses on recovery, and improves community adjustment, functioning and quality of life.

Sublocade: Monthly Buprenorphine injection. Patients need at least seven days on oral Buprenorphine which is provided.
Probuphine: Six-month Buprenorphine implant for patients stabilized on 8mg or less of oral Buprenorphine.

Family Based Mental Health (FBMH) is an in-home family therapy service. The overarching goal of this program is to keep families together. FBMH promotes family unity and works toward reducing the need for out-of-home placement such as psychiatric hospitalizations and residential treatment facilities. A wide range of services are provided by the FBMH team including but not limited to; therapeutic interventions, education and skills building, case management, and family support services. This program is voluntary and time limited to generally not more than 32 weeks depending on the needs of the family. The highly intensive program also offers a 24 hour/day, 7 day/week Hospital Diversion/Crisis Stabilization component available to the families.
