Targeted Case Management provides case management services to individuals with mental illnesses who have been referred by the Philadelphia County Office of Mental Health. ACCESS - West Philly works to improve the quality of life for chronically homeless people in Philadelphia who have both a severe mental illness and an addiction. Outreach workers encourage those who are homeless to visit the ACCESS offices in West Philadelphia. There, other program staff is available to offer other assistance through various public systems.

Utilizes the Child and Adolescent Service System Program (CASSP) to explain, offer, and assist with the delivery of Behavioral Health Services for children and their families. Upon a child being referred, intake worker will assist the child's parent/guardian in one of three ways; a parent self-referring their child to a provider of the Behavioral Health service being sought, an intake with a Northumberland County Behavioral Health caseworker for the purpose of intake worker referring the child to an agreed upon Behavioral Health service, or an interagency team meeting for the purpose of developing a Behavioral Health service plan for the child and subsequently referring the child to a provider.

The Lehigh Valley ACT Permanent Supportive Housing program provides independent, scattered site housing funded through a McKinney-Vento grant to chronically homeless individuals with serious and persistent mental illness. Location Northampton

Critical Time Intervention (CTI)/Case Management it is an evidenced-based practice proven to reduce the number of people returning to institutions once released into the community. Case management for homeless invdividuals with mental illness is provided.

DCBHC, the only free standing children’s psychiatric hospital in Chester County, Pennsylvania, employs a team of acute care professionals dedicated to meeting the needs of children and adolescents ages 5-18, and their families, in acute mental health crisis. Mental Health Case Management focuses on community integration and the life domains of housing, financial benefits, education, employment, physical and mental health, and natural social supports.

There are two levels of support:
- Administrative Case Management: Primarily office-based and less intensive; available to children and adults of Bucks and Montgomery Counties
- Blended Case Management: Primarily community-based and offers a more intensive level of service; available to children and adults of Bucks and Montgomery counties; recovery coaching is a model of Blended Case Management for adults that offers team-delivered services with a clinical consultation component

Community-based intensive case management for children with mental health issues.

Provides the following services to individuals struggling with alcohol or drug use:

Individual, couples, and group therapy

Provides intake and referral services, individual, couples, family and group counseling, comprehensive evaluations, and medication monitoring.

Provides outpatient substance use treatment for adolescents and adults. Services include initial assessment, individual and family therapy, peer recovery support, and case management. Also assists clients with accessing Medication Assisted Treatment (MAT), psychiatric services, or other levels of treatment as appropriate.

Addresses the behavioral concerns of youth and teens related to their behavioral health diagnosis in the home, school, and community. Services include creation and monitoring of individual treatment plan, case management, crisis management assistance, and mobile therapy that conducts weekly sessions in the home or at school. Community Behavioral Health (CBH) funded.

Service in which a strategic treatment plan is developed and implemented with a team approach in an attempt to stabilize a child or adolescent's behavior within their natural environment.

An assessment Unit provides or connects clients with appropriate services. The case management staff provides ongoing support to the individual and the family. Contracted provider agencies offer a variety of general and intensive services in the community. 24-hour crisis intervention also available.

Provides a comprehensive continuum support services network which utilizes a collaborative approach to the provision of mental health services. Services include alternative education, emotional support, partial hospitalization, school-based outpatient program, and Behavioral Health Rehabilitation Services (BHRS).

Provides substance use disorder assessments and referrals to all levels of treatment, substance use disorder recovery support services, warm hand-off (24/7 access to care) at both local emergency departments, mental health case management services, and a urinalysis testing center.

Offers a range of services to children who have been sexually abused. We offer free individual and family therapy, as well as psycho educational and support groups. Additionally, we run the Delaware County Children's Advocacy Center where children referred by Children & Youth Services, law enforcement agencies, or the District Attorney's office are interviewed when abuse is suspected. We provide abuse prevention programs for children, teens, and adults, as well as specialized professional training.

Assists children and adolescents with a variety of behavioral health/medical needs and their families by building a strong network of community resources and services, linking clients with needed services in the community. Provides a variety of supportive services including advocacy for children and their families; negotiation of complex systems such as special education and social services; assistance for families in maintaining and finding access to basic living needs and skills. Provides on-site crisis intervention, with on-call Case Management available 24 hours/7 days a week.

Starting Point program provides case management and support services for adults with mental illness. All programs are community-based.

Behavioral Health Services offers recovery-oriented services for adults and resiliency-oriented services for children.

The Extended Assessment Program offers up to 24 hours of assessment and case management to children in Delaware County. The program links children with the services that will best aid their overall growth and development. A fully-credentialed Master's -level clinician and a clinical case manager oversee each child's case and collaborate with the school, community, and home to determine the most suitable resources for the child. Recommendations may include continued assessment, EPSDT/Wraparound evaluation, or another level of care.

Case management services for adults with behavioral health issues identified as serious mental illness. Case managers develop recovery plans for individual clients, arrange for required services, monitor services and serve as an advocate for the individual.

Offers mental health support services including information/education, intervention, self management and transitional services. Also provides post disaster mental health services.

Provides non-clinical primary mental health services to insure that the most seriously mentally ill individuals receive additional support geared to improving the quality of their lives and reducing psychiatric hospitalization. Casemangers use natural occurring supports in the community, i.e., family, friends and neighbors. Includes professionals in social work, psychology, counseling, and psychiatry who review client cases and make treatment recommendations. The behavioral case manager serves as a liaison connecting the IBHS team and the client’s medical providers and nursing professionals to facilitate comprehensive care.

Offers a Medication-Assisted Treatment (MAT) program for those suffering from substance use disorders. Team consists of two psychiatrists, a Mental Health Nurse Practitioner, a Psychiatric Nurse Case Manager, a Licensed Practical Nurse, a Registered Medical Assistant and several therapists who are Certified Advance Alcohol and Drug Counselors (CAADC). The program is a combination of both FDA-approved medications and counseling. Three types of MAT are offered:
Vivitrol: a monthly opioid blocker for patients free of opioids for at least five days. We can provide an outpatient detox protocol if needed.
Sublocade: Monthly Buprenorphine injection. Patients need at least seven days on oral Buprenorphine which is provided.
Probuphine: Six-month Buprenorphine implant for patients stabilized on 8mg or less of oral Buprenorphine.
Please note, our practice does not prescribe maintenance oral Buprenorphine.

Outpatient Psychiatric Services are offered at school sites throughout the Coatesville Area School District. Services can be received at the following schools: Caln Elementary School, East Fellowfield Elementary, Friendship Elementary, Gordon Elementary, King's Highway Elementary, Rainbow Elementary,Reeceville Elementary, North Brandywine Middle School, S. Horace Scott Middle School, South Brandywine Middle School,9/10 Center, and Coatesville Senior High School.

Case managers assist individuals obtain supports that help reduce hospitalization so that they can continue to live in the community in the least restrictive setting possible. Our case managers help individuals with their housing, day program, appointment scheduling and other daily living needs. Call 215 748 8400 for eligibility and referral information.

The heart of WRC, the Information & Referral Helpline provides connections to community resources, intake for WRC programs and services, and important data on the evolving needs of the women who reach out to WRC for assistance.

Services include high fidelity wraparound, family based mental health, multisystemic therapy, outpatient treatment, blended case management, and mental health respite services.

Assures appropriate services are available for individuals who are experiencing mental health challenges. Contracts with agencies who provide an array of services and resources which are focused around the principles of recovery, resilience and self-determination. Services offered include psychiatric rehabilitation, outpatient treatment, case management, housing, acute inpatient treatment, crisis services, and peer support.

Assures appropriate services are available for transition-age youth who are experiencing mental health challenges. Contracts with agencies who provide an array of services and resources which are focused around trauma informed principles, recovery, resilience, and self-determination. Services include high fidelity wraparound, outpatient treatment, blended case management, transition age housing, and program of assertive community treatment.

Staff meet people with mental health challenges in the settings of their choice and focus on helping people develop skills and access resources needed to reach their recovery goals.
