Information and referral service provides information on the availability of services and programs for older adults. Services may be delivered directly, purchased from a community agency, or available through a cooperative agreement.

Provides in-home treatment program to families in crisis that aim to reduce out-of-home placements and length of unavoidable out-of-home placements. The intensive In-Home Support programs provide individual, couple and family counseling, and assist with daily living skills to improve interaction and functioning.

Diakon's Family-Based Mental Health Services program provides counseling services to individuals and their families, with services primarily occurring in the home, although they can be extended to the school or community setting. The goal of the program is to provide families with the skills and resources necessary to prevent their child or adolescent, who is experiencing significant emotional problems, from requiring out-of-home placement.

Alternative to secure detention

Provides detoxification services.

Substance abuse detoxification and residential rehabilitation for adult men. Treatment services include: Moral Reconation Therapy (MRT), Male Trauma Recovery Empowerment Model (MTREM), anger management, yoga & meditation, Gay Straight Alliance, gardening, drumming and volunteer opportunities.

Facility offers residential detox and addiction rehab services for men and women ages 18 and older. Services use a person-centered, strengths-based approach. The multiple-pathways-to-recovery approach to treatment, includes both Medication Assisted Treatment or an abstinence approach.

Offers mentorship and professional counseling services. Services include needs assessment, navigating ChoiceOne services, community referrals, and family and relationship support.

Inpatient facility staffed by trained medical personnel that provide assistance, support and medication, to help individuals who are physically dependent on substances during withdrawal.

Provides free, developmental screenings for children, birth to three years old. Offers a variety of supportive services for children and their families, including developmental evaluations, physical, occupational and speech therapy, vision and hearing screenings, family education and social work services.

Provides support to women who are pregnant, to families seeking to adopt, and to children in need of a temporary or permanent home when reunification is not an option. Offers assessment of a family's situation, case advocacy, support groups, education, respite, family cam,p and referrals.

Provides mental health services, referrals, case management, children and adolescent services, system program (CASSP), community hospital integration projects, partial hospitalization, inpatient treatment, and peer support.

Families partner with providers and guide them in creating a plan, building on their family's strengths, that will meet the needs of their child and family. Through coaching, families learn how to embed opportunities into their daily routines and activities that will enhance their child's development. The multi-disciplinary team providing services may include a variety of therapies and teachers based on family need. Services support families with children who may have developmental concerns including but not limited to motor, speech, social emotional, feeding, behaviors, medical diagnoses that may result in developmental delays and more.

Provides public school special education services and supports for preschool children with special needs. If a child is found eligible for services, an Individualized Education Program is developed to provide support for the child's participation in typical preschool activities. Supports include training and consultation for the staff working with the child in their early learning program, special instruction, and related therapies. The program provides the supports to enable a child to attend an early learning program, but does not provide the early learning program unless the child's needs can only be met in a preschool special education environment.

Provides facility based integrated activities as well as access of support services. Provider of Lekotek and Compuplay, which are part of service offerings for children with disabilities, utilizes play strategies with adapted toys and computer software, respectively, to further a child's developmental goals. Physical, Occupational, and Speech Therapies are all within UCP of NEPA's EI services only.

Provides outpatient counseling for victims of crime, violence, or abuse. Services include individual counseling, group counseling including the Restoring Families Program, trauma-focused cognitive behavior therapy, support groups, trauma sensitive yoga, and Eye Movement Desensitization and Reprocessing (EMDR).

Provides therapy, recreation, meals and support for those seniors who require constant supervision which their families are unable to provide, or for those seniors who need socialization opportunities but are too physically or mentally impaired to attend a senior center, Adult Day Care provides daytime supervision and activities which are geared to the abilities of the participants. This service may be subsidized by the Area Agency on Aging.

Provides inpatient detoxification from a variety of substances including heroin, prescription opiates (Percocet, OxyContin, Vicodin), benzodiazepines (Xanax, Klonopin), alcohol, and other drugs of abuse in an non-hospital, medically monitored setting. Individuals will meet with a physician daily and will have access to a psychiatrist as needed.

Supports children to reach their academic and social potential in school settings through comprehensive behavioral health services within schools throughout Philadelphia. Services include individual therapy, and collaboration with caregivers and school staff to address behavioral concerns both in and out of the classroom.

Enables people with any life-limiting illness to remain comfortable in familiar surroundings, with support of family and friends. The programs offers the most advanced therapies, medications, counseling, personal services and palliative care for symptom management and pain control. Patients may arrange hospice care at home, in a nursing home, assisted living facility, local contracted hospitals or Holy Redeemer Inpatient Hospice Unit.
