Adult Care of Chester County believes that a comprehensive adult day services program includes services specific to the strengths and needs of the people attending. That includes nursing oversight, medication administration, and other skilled nursing services that match the needs of each individual. It includes assistance with activities of daily living at the appropriate level for each person attending. It includes therapeutic activities that are carried out with modifications to match the abilities of each person in the group. It includes a full-time music therapist on staff to provide music therapy and development or enhancement of hidden talents and interests. Lunch and two snacks are provided with special diets based on nutritional and medical needs of the people attending. Our comprehensive adult day services program also includes caring for the family caregivers through individual and group support and education. People attend 2-6 days per week depending on their needs and circumstances.

Program participants may learn shopping skills or daily living skills in a practical setting through weekly trips to local stores within the immediate community. Inclusion is promoted through volunteer efforts, including delivering Meals-on-Wheels, volunteering at local animal shelters, or cleaning up local parks and playgrounds. These activities are designed to give the community a chance to meet and interact with the ATF program participants. Community Supports Option Also provides entirely community based programing to those seeing a non-traditional setting. The CSO staff to consumer ratio is 1:3. The individuals participating in this program do not attend a facility based program. Their activities are carried out in the community 5 days a week. This program focuses on volunteering and practicing community related skills.

    Home Safe Home provides accessibility assessments and projects such as grab bars, ramps, and stair glides. cost of projects may have a co-pay. These services provide in-home assistance with activities of daily living, such as bathing, grooming, and mobility for persons with chronic health problems. Homemaker Service: For those receiving personal care services from BCOA, shopping, laundry, light housekeeping and other services may be available when health problems prevent maintenance of household activities. Respite Care provides relief to families caring for and living with an elderly homebound person for up to four hours a week and can provide personal care if needed. Adult Daily Living provides day programs for the frail elderly with physical and/ or mental impairment. Provides respite for family caregivers. Day Care centers are open daily and provide supervision, meals, and activity. Chore Service is a limited service available only to existing BCOA consumers. It provides assistance with minor home repairs. The consumer must pay for supplies. Ombudsman Service investigates and resolves complaints made by or on behalf of residents in nursing homes, personal care boarding homes and Domiciliary care homes. Complaints may include any issue that affects the health, safety, welfare and rights of a resident. Prime Time Health program provides community-based programs on health-related issues. Topics are intended to promote the benefits of a healthy lifestyle. Available in the Senior Community Centers. Protective Service responds to reports of suspected abuse, neglect, exploitation, abandonment, inappropriate relocation and emergency/ crisis situations. A case manager investigates the report and provides any necessary intervention.

Airy Avenue Philadelphia, PA 19119 Participant Profile Persons who enroll in our adult day center are adults, generally over the age of 60, who are having difficulty managing everyday activities on their own. Center participants need assistance to varying degrees with performing routine self-care tasks (bathing, dressing, grooming) as well as home management (housekeeping, laundry, and meal preparation). Recreation and Leisure Program The focus of the adult day center is on wellness through recreation and leisure. Programs are designed to restore, strengthen, and maintain well being. The benefits of therapeutic recreation include opportunities for: cognitive and physical stimulation; peer interaction; community involvement; building self-esteem and self-worth; gaining or possibly rekindling past interests; and, identifying methods of self-expression. Our structured program helps to reduce anxiety and brings comfort through a sense of belonging. Professional Services Nursing care is provided by an RN. The primary goal is to optimize the overall health and safety of the participants, and to educate and support caregivers. Participants are experiencing a broad range of complex, chronic health conditions including dementia, heart disease, diabetes, arthritis, and Parkinson's disease. Many participants are memory impaired. Direct care staff provide hands on support throughout the day. When an acute health episode results in hospitalization, the Social Worker and RN work closely with the primary physician and other professionals to plan for their discharge and monitor the participant's progress at home. In addition, Homelink assists caregivers with identifying primary care physicians including home visiting doctors specializing in geriatric care. The adult day center provides a respite resource to families and other caregivers. Information and referral, counseling and long term care planning are provided. Staff help to arrange for care at home and respite in a nursing home or assisted living facility. The monthly newsletter includes resource information and workshop announcements. Podiatric care is provided on site, helping to further reduce the demands on caregivers. Transport Some participants are transported to and from the center by caregivers while most use SEPTA ParaTransit. Staff work closely with caregivers and public funding sources to facilitate access to transportation services for both adult day center transport as well as travel to medical appointments. Payment The daily charge for adult day participation is paid by the consumer, caregiver or a public funding source. Homelink, Inc. is an Options and Waiver provider for the Philadelphia Corporation for Aging. In addition, the Philadelphia Department of Veterans Affairs supports eligible veterans in the center. Participants and their caregivers may choose to pay for additional days of attendance when their needs and interests exceed the resources available through public funding sources. Services are provided and admissions and referrals made without regard to race, sex, color, national origin, ancestry, religious creed, disability, age or sexual orientation.
