Nursing home placement for individuals who can no longer stay safely on their homes and who require 24 hour supervision.

Provides lcoations for nursing home facilities.

Provides support and services to enable older adults to continue to live in their homes and communities. Services may include Care Management, Assessment, Protective Services, Information and Assistance, PA MEDI (Insurance Counseling), Health and Wellness Programs, adult day services, in home meals, personal emergency response system, personal care, transportation, and caregiver support. The agency also has 5 Senior Centers throughout the County in which participants can enjoy a Congregate Meal and Programming.

Provides information, referral, and assistance relating to services and benefits for older adults such as: assessing, developing and maintaining care plans, assist in completion of forms including PACE/PACENET (programs to help with the cost of medicine & prescriptions); helping to understand Medicare and Medicaid benefits; info on Senior Centers and Day Programs; transportation services; reporting when there are abuse concerns; and In-Home services such as personal care, respite care, chores and/or home delivered meals.
