Provide readjustment and transition counseling to veterans of military campaigns and their family members. Services available at Vet Centers include individual, group and family counseling, bereavement counseling for families of an active duty death and counseling for military sexual trauma.

Call centers provide information on Vet Center locations and referral for all counseling services, as well as substance abuse, medical issues and employment services. A suicide hotline is available 24 hours a day.

Dental problems, such as pain and/or missing teeth can be tremendous barriers in seeking and obtaining employment. Studies have shown that after dental care, Veterans report significant improvement in perceived oral health, general health and overall self-esteem, thus, supporting the notion that dental care is an important aspect of the overall concept of homeless rehabilitation.

VA Pittsburgh Healthcare System's Community Support Program/Mental Health Intensive Case Management Program (MHICM)works with veterans diagnosed with serious mental illness, helping them transition to community living and fostering independence and recovery.

MHICM advocates a philosophy of recovery and rehabilitation. Veterans enrolled in this program have an assigned intensive case manager who has frequent contact with them through regular visits. The case manager will facilitate their integration into the community using natural support systems, such as family, landlords and employers, whenever possible.

Our integrated, multidisciplinary team delivers services that provides intensive, flexible community support, improves health status by reducing psychiatric symptoms, and decreasing inpatient hospital use, focuses on recovery, and improves community adjustment, functioning and quality of life.

3. Have financial resources to support community living,

4. Reside within 50 miles of VA PHS and are not a danger to
themselves or others.

The H.O.M.E program provides transitional housing, on-site case management, food, clothing, transportation, child care subsidy/assistance, employment support and other essential supportive services to homeless women Veterans and their children.

The H.O.M.E program focuses on integrating women Veterans back into their local communities and providing vast, safe and suitable residential areas to choose from.

The H.O.M.E program is a collaboration with our resource partners that includes targeted supportive services to assist women Veterans in reaching their goals.

Although based in the DC Metro Area, the program has assisted women veterans and children in over 15 States and Territories.

A program working with people experiencing homelessness having the express goal of moving to permanent housing. All programs are housed at 105 E King St. Within the program, they serve many populations, including but not restricted to Veterans through our Veterans’ Victory House, families and households through the Emergency Shelter program.

Each household is assigned a case manager who works with each individual or family to develop an Individualized Housing Plan, again with the ultimate goal of achieving permanent housing. The plan, worked on together with the family or individual includes, among other items, a budget and a potential referral to a rapid re-housing program. Emphasis may also be placed on education, employment and/or recovery needs.
