The transition from the military life to civilian life starts with a good plan and at the top of the list of things to complete is finding a job. Aims to provide the growing population of Post 9/11 Veterans calling the Greater Pittsburgh and Western Pennsylvania area home with assistance in achieving this goal by providing resources and information about job openings, fairs, and other resources here in the area.

Steel City Vets also works with companies in the area wanting to hire Veterans. We can provide these companies with resources and information. Steel City Vets is honored to work along side employers and Veterans in an effort to help reduce the number of Veterans looking for a job.

Provides emergency assistance for adults with a history of service in the military in the home or community.

Phoenix works with the Veterans Leadership Program to provide three rooms of furniture to disabled veterans. Furniture is also provided and delivered at a minimal cost to those in need in the community. Donations of appliances and furniture are appreciated. Pickup is provided for those who have articles to donate to the program.

Purchases, constructs, renovates or improves homes that are sold to qualified applicants. Also assists veterans with special needs to improve their quality of life.

Provides a supportive housing program for female veterans and their children that includes case management and peer support.

Provides elderly or chronically ill veterans with coordinated care that focuses on individual needs, enabling each patient to enjoy the highest quality of life possible. Services that are provided fall into the following categories: Hospice and Palliative Care, Home Health Care and Adult Day Health Care, Community Living Center Care and our outpatient Golden Memory Clinic.

Program provides financial reimbursement for disposable medical supplies, services, home modifications, and adaptive devices as well as other supportive services to caregivers of aging relatives and grandparents raising grandchildren.

Ombudsmen work to resolve complaints and issues on behalf of individuals residing in long term care settings, such as nursing homes, assisted living facilities, and personal care homes. They help to educate residents on their rights and advocate on their behalf.

• Monthly stipend,

The 196-bed Gino J. Merli Veterans Center provides a broad spectrum of health care and related services to those commonwealth veterans and their eligible spouses who are disabled, chronically ill, or in need of specialized care. We assist residents in functioning independently and at their optimal level. Most importantly, all residents are treated with respect and dignity.

Our facility includes a chapel, pharmacy, barber and beautician services, dining rooms, and a multi-purpose room for large gatherings. Outdoor areas include a courtyard with a covered pavilion and a 'victory' garden area where residents plant flowers and vegetables.

A calling program for deployed service members that provides free phone time to active-duty military personnel and hospitalized veterans.Provides at least three designated days each month when deployed service members can make FREE phone calls home to the U.S. from their Morale, Welfare & Recreation (MWR) center or military DSN line.

Free Call Days have provided more than 7.5 million free connections for service members and their families.

VFW Unmet Needs was created in 2004 through a corporate partnership to assist military service members and their families who run into unexpected financial difficulties as a result of deployment or other hardships directly related to military service. Unmet Needs assists with basic life needs such as: mortgage and rent, home and auto repairs, insurance, utilities, food and clothing. Unmet Needs helps meet unanticipated financial demands on our service members' families that can't be remedied through existing means and provides our service members with the security of knowing that their families have additional support here at home.

The financial assistance is in the form of up to $2,500 in grants that do not need to be repaid. All grants are paid directly to the "creditor" (such as the electric company), and not to the individual. Each case is reviewed individually and acceptance determined by a committee.

The National Amputation Foundation (NAF) has for over 80 years been offering valuable assistance to veterans of World War I, II, Korea, the Vietnam Conflict, Desert Storm and Iraqi Freedom. Since then, the Foundation has expanded its facilities to include civilian amputees as well.

The National Amputation Foundation has programs and services geared to help the amputee and other disabled people.

AMP to AMP Program
A home, hospital or nursing home visit is available for peer counseling and support to any person who has had or will be having a major limb amputation. If the person does not live within a drivable distance, we will call them to offer the same support.

Medical Equipment Give-A-Way Program:
Our donated medical equipment program is available to any person in need. This includes wheelchairs, walkers, commodes, canes and crutches. While this program is open to anyone in need, the item(s) need to be picked up at our office.

MFH Veterans have complex medical conditions where personalized care and assistance are needed. MFH is an affordable alternative to nursing homes or assisted living facilities. VA Pittsburgh Healthcare System's Adult Day Health Care is a therapeutically-oriented outpatient program that provides health maintenance, rehabilitative services, socialization, and caregiver support for veterans in a group setting.

An interdisciplinary team of health .professionals and support staff help participants and their caregivers develop the knowledge and skills necessary to manage care in their home.

The dementia program for the memory impaired is located in a secure room; the general program is located in an open, monitored room.

VA Pittsburgh Healthcare System's Community Residential Care (CRC) program provides health care supervision to eligible Veterans who do not need acute hospital or skilled nursing home care but are unable to live independently due to mental health conditions or long-term chronic diseases and do not have family to provide such care.

CRC provides:

• Assistance with housing (Veterans can choose from residential group homes within 60 miles of the University Drive campus that are inspected annually by the CRC team),
• Minimal assistance with activities of daily living,
• 24-hour supervision as determined on an individual basis.

VA Pittsburgh Healthcare System's Home Based Primary Care program (HBPC) provides primary and tailored health care and skilled nursing services for the home bound, chronically ill, and frail elderly Veteran population within Allegheny County

The HBPC interdisciplinary team provides medical and psychiatric care, nurse case management, clinical social work services, rehabilitation services, dietician and pharmacy services. All interventions are designed to meet the Veterans' individual needs in order to allow them to remain in their homes and manage their chronic disease processes. Veterans who cannot safely live alone must have a caregiver who is willing and available to assist in their care.

VA Pittsburgh Healthcare System's Homemaker/Home Health Aide Program provides non-skilled services to Veterans, in need of assistance with daily activities of living (ADL's), in their home, as an alternative to placement while contributing to an improved quality of life for the Veteran and their caregiver.

Veterans being followed by a VAPHS Primary Care Provider with medical/psychiatric conditions making it difficult to maintain independence maybe eligible for non-skilled services for ADL assistance such as bathing, grooming, toileting and dressing. If meeting criteria for ADL assistance, Veterans may also qualify for assistance with light housekeeping and limited in-home respite services. Services are authorized through the Homemaker/ Home Health Aide Program and provided by community homecare agencies.

VA Pittsburgh Healthcare System's Contract Community Nursing Home Program (CNH) coordinates nursing home placement in community nursing homes for qualifying Veterans.

Participation in the Contract Community Nursing Home Program requires that a veteran be admitted to a VA contract nursing home. This program falls under Veterans Millennium Health Care Law 106-117: âCommunity Nursing Home care may be provided to veterans who are in need of such care for a service-connected disability and have a service-connected disability rated 60% or more.â If a veteran does not meet the above criteria, a social worker can determine if funding is available for a short-term contract. Veterans under CNH contract are visited every 30 days by a CNH nurse or social worker. The CNH team assists with discharge planning, coordination of VA follow-up appointments, and overall monitoring of the quality of care provided in the contract nursing home. In addition, contracted facilities undergo a yearly review to ensure that t

VA Pittsburgh Healthcare System's Community Health Nurse (CHN) Program coordinators maintain a centralized referral program which involves assessing impatient and outpatients for home health skilled nursing needs.

CHN staff explores availability of community skilled nursing services with each patient referred for home care services. They complete and forward nursing care referrals and provide on-going liaison services between the home health agency and VAPHS health care providers.

Social workers in primary care are a part of the VA Patient Aligned Care Team (PACT) initiative which involves a comprehensive approach to care in conjunction with other medical center disciplines.

Social workers provide comprehensive clinical social work intervention with patients, families, and caregivers. Types of interventions include, but are not limited to, psychosocial support intervention, crisis intervention, risk assessments, advance directive education and preparations, resource identification/linkage (both VA and non-VA), placement procurement into nursing homes for those patients requiring skilled care, respite placements, transportation resources, assistance with homeless needs and advocacy within the VA system for our patients.

VA Pittsburgh Healthcare System's Home Telehealth program provides chronic disease management through the daily use of Telehealth technologies.

Veterans diagnosed with chronic diseases (such as diabetes, hypertension, kidney disease, heart failure, chronic obstructive pulmonary disease, post-traumatic stress disorder, depression, and substance use disorder) who meet enrollment criteria receive a home monitoring device which transmits information such as blood pressure, weight, glucose readings, and mood to a Home Telehealth nurse s that medical and psychiatric conditions can be closely monitored. Veterans can participate by using a land line telephone, their personal cell phone, the internet, or via a cellular modem provided by VA. The Home Telehealth program improves communication with the veteran and medical team to improve symptom management and decrease emergency room visits, hospital admissions and long-term nursing home placement.

VA Pittsburgh Healthcare System's Caregiver Support Program is designed to provide support to family caregivers of veterans.

There are many available services and supports through VAPHS and the national VA caregiver support program, including a specialized program for caregivers of eligible veterans who were seriously injured or psychologically injured in the line of duty on or after 9/11 (go to www.caregiver.va.gov for more information). In addition, VAPHS his many home and community-based care programs that are designed to promote the care of the veteran at home by providing the caregiver assistance. There are also various support groups and educational opportunities available.

The residents, staff, and volunteers of the Pennsylvania Soldiers' and Sailors' Home extend a warm welcome to you.

Our home is situated on 26 pastoral acres overlooking Lake Erie and is steeped in history. Remodeled and modernized, our home provides state-of-the-art care and treatment in a homelike atmosphere.

A little-known fact is that spouses of veterans are eligible for admission. Merchant Marines with "oceangoing" service between December 1941 and August 1945 and veterans of the Pennsylvania National Guard are also eligible for admission.

The Delaware Valley Veterans Home was dedicated on November 1, 2002 and is the commonwealth's sixth and newest veterans home operated by the PA Department of Military and Veterans Affairs. Our facility serves 171 veterans and their eligible spouses in a caring and home-like setting. It is a spacious facility with skylights and large windows throughout the building.

We are located just minutes from I-95 and the Pennsylvania Turnpike in Northeast Philadelphia at the border of Bucks County. The home offers a rural setting despite its city location with meadow and woodland views all around. Adding to the tranquil setting are the frequent sightings of deer, hawks and a variety of other wildlife.

Federal benefits include:

Delivers meals to Veterans' homes in Erie county. Veteran receives one ready-to-heat meal per day, Monday through Friday (frozen weekend meals delivered on Friday) by a volunteer.

Provides assistance with eating, bathing, shaving and other activities of daily living which do not require constant nursing care. The nursing unit provides 24-hour care, seven days a week. Residents receive a complete range of clinical services under the direction of physicians, nurses and other licensed health care professionals.
The 32-bed Dementia/Alzheimer's unit provides veterans suffering from dementia or Alzheimer's disease with long-term care in a safe and secure environment.

Provides independent living programs and direct care services to all individuals with disabilities. Also provides autism services, veteran services and Hospice Wraparound services.

VA loan guaranties are made to service members, veterans, reservists, and unmarried surviving spouses for the purchase of homes, condominiums, and manufactured homes and for refinancing home loans. VA guarantees part of the total loan, permitting the purchaser to obtain a mortgage with a competitive interest rate, even without a down payment, if the lender agrees. The guaranty means the lender is protected against loss if you fail to repay the loan. Applicants must have a good credit rating, have sufficient income to support the mortgage payments and agree to live in the property as their primary residence.

Franklin County pays burial benefits for war-time veterans and the widows at the time of death; and the installation of a standard government headstone or marker. The amount of the benefit is $100.00 towards the burial and $50.00 towards the installation of the headstone/marker. It is customary for the funeral homes, in Franklin County, to file for this benefit at the time of the veteran's/widow's death.
Franklin County also supplies flag holders/flags for cemetery plots of deceased war-time veterans within the county. The flags holders are placed at the grave at the time of burial. Every year Franklin County purchases and distributes, through volunteer coordination, flags for honoring veterans for Memorial Day; however those flags are then taken off the graves the first business day after the July 4th holiday (at the discretion of the individual cemeteries).

Point Man International Ministries helps those who have served in the military with recovering from post traumatic stress disorder (PTSD). A system of small groups, known as Outposts, are run across the USA for the purpose of mutual support and fellowship. Also offers Home Front support groups lead by Christian mothers, wives and friends of both active duty military and veterans. Acts as a referral service to connect hurting veterans and their families to the Outpost and Home Front system for continued support and fellowship.

The Veterans Affairs Department is responsible for providing assistance, counseling and representation to veterans and their families in accessing county, state and federal benefits.
Services of this office include:
Burial assistance.
Records maintenance.
Providing grave markers, flags and holders for all county veterans’ graves.
Financial assistance to county cemeteries holding veterans.

Tax exemption and educational assistance for 100 percent disabled veterans.
Emergency assistance.
Applications for the blind (paralyzed veterans and veteran’s homes).
* Federal veterans benefits applied for through this office include:
Health care.
Service connected compensation.
Pension assistance for disabled, unemployable veterans.
Counseling for surviving family members and returning veterans.

Assists homeowners, including veterans, seniors, and disabled homeowners with minor exterior home repairs. Typical projects include exterior painting, accessibility modifications (ramps, handrails, etc.), fence replacement, deck and stair, and overgrown brush removal. WE DO NOT REPLACE/REPAIR ROOFS.

Services of this office include burial assistance, records maintenance, providing grave markers, flags and holders for all County veterans graves and financial assistance to county cemeteries holding veterans. State benefits applied for through this office include tax exemption and educational assistance for 100% disabled veterans, emergency assistance, applications for the blind, paralyzed veterans and veterans homes. Federal Veterans Benefits applied for through this office include health care, service connected compensation, pension assistance for disabled unemployable veterans and counseling for surviving family members and returning veterans.

Beacon House is sponsored by the "Veterans of Vietnam War Post #30" and administered totally by volunteer efforts. Provides temporary housing for homeless veterans. The house serves as a transitional group home for up to eight homeless veterans who have completed drug and alcohol recovery programs at Veterans Administration Hospital in Lebanon. Counseling and support are also provided to veterans. Resident veterans are provided with medical and vocational rehabilitation services through the Lebanon Veterans Medical Center. Volunteers also help with driving and maintenance of the agency van.

Speakers are available to give presentations about the organization to interested groups.

Assist veterans and their family members with information regarding federal, state and local benefits. Provides information and assistance in filing for and acquiring the following federal benefits: compensation for service connected disability; pension for veterans unable to work; vocational rehabilitation and counseling; payment for education and job training; home loan guaranty; life insurance; "wheelchair homes"; and burial benefits. The benefits for family members of deceased veterans include dependency and indemnity compensation and death pension.
