Provides support for people with serious mental illness who are living independently in the community. Staff work one-on-one with participants to develop a personalized service plan to provide the support a person needs to maintain independent living.

Offers residential services for physically and/or cognitively disabled adults. Program is run by the Allegheny County Human Services Department, Family and Community Services.

Provides community based program housing for intellectually disabled adult individuals in "group home" settings. Those with physical challenges are accommodated fully. The program is licensed to operate and monitored regularly by the Pennsylvania Department of Human Services.

Shared/Supportive Housing is a peer support based housing option for individuals of Butler County who are afflicted with persistent symptoms of mental illness, but wish to return to the least restrictive form of independent and community based housing. Supportive Housing is often used for individuals who graduate from the Partial Care CRR but are still in need of a reduced level of support to maintain within the community. Common intervention includes landlord/tenant relations, conflict resolution with peers/roommates, liaison with utility providers and treatment team, etc.

Through supportive/shared housing, residents form a natural support system of their peers. Staff is available 24/7 for conflict resolution and independent living skills support and training. Staff monitors involvement in community resources to ensure that residents receive the maximum level of available benefits/supports. Staff networks with local and reputable landlords to ensure residents are offered a fair lease and secure a sound,

Sexual Trauma Treatment and Recovery services
Provides counseling, advocacy, and education to sexual violence survivors of all ages and their significant others. Provides a 24 hour sexual assault hotline. Services include individual, group, and play counseling; prevention education, and legal and medical advocacy.

Residential facilities are located throughout Lancaster County.

Practice for families to gather and create their own plan in regards to the reunification of their family after a child/children were removed from the home. Families meet with relatives, friends, case management and providers to discuss strengths and concerns. The family is given private time to create a plan that will help protect and prevent future harm to their children.

Provides a toll-free crisis hotline, emergency shelter, individual and/or group counseling, and support groups for adults and children. Offers legal advocacy (including obtaining a Protection From Abuse Order), systems advocacy, children's program, information and referral, and accompaniment to court-related proceedings.

Provides permanent, supportive, group residences for individuals with serious and persistent mental illness who were formerly institutionalized. Program services include access to health care services, medical nursing services, medication management, recreation activities, case management, life skills training, and other community resources.

Staffed primarily by people with the lived experience of mental health and co-occurring challenges. Our direct service includes both individual and group peer support. Additionally CIC coordinates the training institute for the Montgomery Co. Office of Behavioral Health. The mission of the training institute is to increase the competency of our workforce particularly in delivering recovery best practices and in evidence-based treatments.

New Options is a licensed personal care home that promotes clients' independence and integrity by providing a supportive community environment for elderly individuals who are mentally impaired, and who had been institutionalized. Clients are encouraged to function as independently as possible via making decisions and choices on activities of daily living and community participation.

Operates 23 Community Living Arrangements (CLA) and one Community Residential Rehabilitation home (CRRH).

Provides supports and services to help people live in their community as completely and independently as they desire.

Offers units specifically designed for individuals with disabilities. Eligible applicants will also receive wrap-around case management services.

A comprehensive information and referral program. The primary service is assessment of caller needs, followed by sharing of information about and referrals to appropriate resources that will address client needs, questions and concerns. Maintains an extensive database of health and human services, including non-profit agencies and government programs.

- Production of the Where to Call Guide, a quick reference booklet of Lancaster County resources.
- Website that showcases resources for each county served by PA 211 East
- Compilation of seasonal resources, i.e. holiday and utility programs
- Centralized intake for anyone experiencing homelessness in Lancaster County

Provides safe house for abused persons who are in immediate danger; case management; service and safety plan development; clothing; individual counseling; support groups; life skills and information workshops; and referrals to and advocacy with other providers.

Provides information on registration and licensing of child care providers and also handles complaints about child care providers and additional technical assistance.

Conducts health assessment of the county every three years and oversees 17 task forces and committees that are addressing past and current community health and human service needs.

Offers affordable rental housing for low-income seniors ages 62 and older and adults ages 18 and older with disabilities. Occupancy is limited to a maximum of two persons per bedroom.

Provides a homelike living arrangement in the community. Dom Care providers open up their homes to individuals who need supervision, support and encouragement in a family-like setting.

Offers cost-effective, non-medical housing model to address many of the barriers that senior citizens may face. Provides companionship and security to individuals. Shared housing may also help with the financial burden of living alone. Provides a mutual benefit for the Host (Individual wishing to share their home) and the Seeker (Individual moving into the home).

Keeping children safe and respectfully engage families by using community-based services and support in order to assist families to successfully care for their children.

Works to prevent and respond to civil tension and violence arising from conflicts between ethnic or cultural groups and public expressions of bias or hate. Tracks reported bias-related incidents around PA to inform the PA Interagency Task Force.
