Helps young adults in foster care (or young adults who have had experience in foster care) transition to adulthood. Youth may be able to receive services to live independently and develop life skills while planning for the future. If a youth has aged out of the services after being in an out-of-home-care placement with the Office of Children, Youth and Families, he/she may reconnect with supports through the Transition Age Youth network. Services are provided by Education Liaisons and Transition Planning Caseworkers.

Eligibility: Individuals age 14 to 24 who have had experience in foster care for at least 30 days after their 14th birthday.
Hours: General Office Hours: 8:30am to 4:30pm.
Hotline answers 24/7 and a contingent of staff is available after hours.
