SPHS Connect - Coordinated Entry
Provides emergency shelter, homelessness prevention, permanent supportive housing, transitional housing, and educational support. Permanent supportive housing and shelter plus care programs include intensive case management. Agency serves as assessment office for intake into western PA continuum of care housing services.

Eligibility: Individuals and families experiencing homelessness. For permanent supportive housing or shelter-plus care, individuals must be experiencing homeless and have a long term disability.
