Intake Requirements: 1) Proof of identity for each applicant

2) Proof of income for all household residents

3) Proof of Pennsylvania residency

4) Immunization record for all children up to age 2

5) Bring all children under age 5

6) If applicable, bring proof of pregnancy from doctor stating due date
