Eligibility: Women who are pregnant, breastfeeding or recently had a baby, infants and children under age 5.

Intake Procedure: Call office to schedule an appointment.
Intake Requirements: Proof of identity for each applicant; Proof of income for all household residents; Proof of Pennsylvania residency; Immunization record for all children up to age 2; Bring all children under age 5; If applicable, bring proof of pregnancy from doctor stating due date
