Pottstown - Project HOPE - HIV/AIDS Outreach Prevention Education - Pottstown
Provides medical case management services to persons living with HIV/AIDS and persons at-risk for the disease, including food and nutrition assistance, transportation assistance, psychosocial and mental health resources, dental assistance, and housing assistance (HOPWA).

Also provides HIV and STD testing and programming for prevention, education, and outreach throughout Montgomery County.
