Provides Blended/Intensive Case management and Resource Coordination. Case managers serve children and adults who have a serious and persistent mental illness which interferes with their capacity to function in significant areas of daily life over a prolonged period of time.
Case managers provide consumers with information about appropriate community services and resources and support them in accessing these services. Services include psychiatric evaluation, medication monitoring, service Coordination, psychiatrist supports, crisis intervention, social skills training, psychiatric rehabilitation, vocational assessment, housing supports, activities of daily living, peer support services, and leisure time training.
