Northeast Counseling Nanticoke - Forensic Assertive Community Treatment (FACT)
The FACT Team is specifically designed for those clients meeting the several criteria for ACT but have a forensic criminal justice history. Services include psychiatric evaluation, medication monitoring, service coordination,
psychiatrist supports, crisis intervention, social skills training, psychiatric rehabilitation, vocational assessment, housing supports, activities of daily living, peer support services, leisure time training, service coordination, crisis assessment, intervention
symptom assessment/management, medication administration/monitoring, and Other Major Supports.

Eligibility: Individuals age 18 and older with serious and persistent mental illness and co-occurring disorders with a forensic criminal justice history.
