Services are provided in individuals' homes and in the community. 1 hour per week up to more intensive scheduling based off the individual's needs.

Adults 18 years and older with serious and pervasive mental health
diagnosis needing assistance in obtaining or maintaining safe and
permanent housing. Must have income.
