- 65 years of age or over as of December 31 of previous year
- Reside with spouse who is age 65 or over
- A widow or widower, age 50-64, as of December 31 of previous year
- Be permanently disabled and age 18-64 during previous year
- Have income of less than $35,000 for the previous year
