Pennsylvania Department of Health - Head Injury Program - Rehabilitation Services
Provides funds for services through a contracted provider to help individuals with a traumatic brain injury (TBI) live independently in their homes and communities. Services include:
  • Pre-enrollment assistance
  • Pre-admission assessment
  • Service plan development
  • Rehabilitation
  • Case management
Services are provided in residential facilities, day facilities (outpatient), and other home- and community-based settings.

Eligibility:
  • U.S. citizen
  • PA resident at time of injury and application
  • traumatic brain injury after July 2, 1985
  • have exhausted all resources for head injury care
  • be 21 years of age or older
  • meet income guidelines
  • complete an application
  • be assessed to determine that needs can be addressed by the Head Injury Program through rehabilitation.
Hours: Monday through Friday, 8:30 am to 4:00 pm
