SNAP Outreach Associates pre-screen households through a confidential, bi-lingual, toll-free number (877-999-5964) to determine potential eligibility and estimate a households SNAP benefit amount. Assistance with filling out and submitting a SNAP application is then provided. After the screening is completed, assistance with filling out and submitting the application is given over the phone. The SNAP team will also provide next steps guidance and follow-up calls for additional support and assistance with the approval process. Other food resources will also be provided if needed, such as food pantries, senior food boxes and congregate meals.
