    Home-based services are those services offered to an individual residing at home or in an alternative setting. The purpose of home-based services is to offer supports to an individual and, in many instances, to his or her family. These supports enable and empower the individual to develop or maintain skills necessary in every day living.

    The individual and his or her support team, including the support coordinator, home-based coordinator, and family or provider, determine specific services to be provided. Outcomes or goals are person-centered, meaning plans are developed based on the needs of each individual. Some outcomes, for example, may focus on increasing participation in community events or to develop social skills. Some outcomes are community based while others are specific to in-home needs.

    CRR services are designed to provide intensive support to assist individuals to develop the practical skills they need to live as independently in the community as possible. Persons live together in a house with their own bedroom and shared living space. They are offered training in skills such as cooking, cleaning, budgeting, vocational and educational pursuits, time management, and medication management. Trained staff is available in the home 24 hours a day.

    New Visions has two CRRs, one with eight bedrooms in Chambersburg and one with 10 bedrooms in Shippensburg.

    Supportive Living services provide support to individuals living in community apartments. New Visions provides two levels of Supportive Living services. More intensive services are provided in Shippensburg and Carlisle where New Visions maintains a staff apartment in the same building as supported apartments so that there is staff available 24 hours a day. Individuals may also receive a less intensive level of support in Chambersburg, Waynesboro, Shippensburg, and Carlisle in their own apartments in the community where they have as much or little contact with staff as they need. In Newville, military veterans who have previously been homeless receive Supportive Living services in a 6-bedroom home with shared living space.

    Individuals receiving Supportive Living services work closely with a case manager to develop goals for independent living. The case managers also can attend important meetings and provide some transportation to appointments. Services should be accessed through the County Base Services Unit (BSU).

    Fairweather Lodge is a home where individuals establish their own support system which enables them to live independently in the community. Lodge members meet together regularly as a group where they develop problem-solving skills, living skills, leadership skills, and develop group decision making. They also work in the Lodge business, earning an income and making decisions for the business.

    Lodge members receive support from Lodge Coordinators who act in an advisory capacity, giving advice, teaching skills, providing information, offering employment assistance, and providing limited transportation services. The Lodge Coordinators work with Lodge members for 40 hours a week and are also available on-call 24 hours per day.
