Services for Older Persons provides care management and overall guidance to persons age 65 and older to help them maintain maximum personal independence in their own homes. Also helps older adults locate quality assisted living and nursing home placements. Services offered in the five-county, greater Philadelphia region.

Offers a wide range of support services to older Lancaster County residents with the goal of allowing them to maintain their independence and self-sufficiency. Most services available to persons ages 60 and over, except for the 55+ Job Bank, Job Bank Workshop and Senior Community Services Employment Program which begins at age 55 and older. Domiciliary Care, Long Term Care Assessments, Ombudsman and Caregiver Support Program available for persons 18 years and older.

Specializes in providing mobile assessments for older individuals who are experiencing a change in mental status, behavior or mood. Can be completed in a familiar setting at the convenience of the individual and loved ones.

Staff members are trained in geriatric care and patients are given complete cognitive and physical assessments in addition to the specific medical treatment called for upon hospital admission.

Provide geriatric assessment of memory issues, dementia, medication reviews, falls, behavioral issues, frailty and maximizing functional ability. Services include geriatric assessments, medication reviews, memory evaluations and preventive care.

Provides primary care medical services ranging from pediatric to geriatric care. Accepts most third party payments. Patients without insurance asked to pay whatever they can. Home visits to homebound elderly available. Also offers personal counseling and smoking cessation programs.

Provides a free assessment administered by a care manager to assess a consumer's needs and appropriate care alternatives.

Participants will meet with a physician and a social service person for an evaluation of their physical, emotional and cognitive health. The senior will be given a list of recommendations that the team has made to share with their personal physician. The purpose of the program is to educate seniors and their families about services and programs that can help them continue to be active and independent in their communities.

A comprehensive assessment, education, support and rehabilitation program that identifies and treats older adults who are at risk for four of the most common geriatric syndromes
- Depression
- Dementia
- Falls risk
- Incontinence

These common syndromes may threaten the ability to live safely at home-leading to premature placement in a nursing or long-term care facility and frequent hospitalization. Our goal is to allow those who are aging at home and at-risk for these conditions to remain as independent as possible and to live safely and comfortably in their homes.

St. Mary Medical Center's partnership with Catholic Housing & Community Services has established the Family Caregiver Center to connect seniors to aging in place resources and services. Our guidance provides older adults and their families with choices, recommendations, and answers in times of uncertainity.

Geriatric outpatient assessment center; that provides consultations, evaluations, and recommendations based on the individuals medical history, health condition, current medicines and needs.

Completes assessment to determine what level of care (i.e. nursing home, personal care home or community services) is appropriate for individuals on Medical Assistance to pay for a nursing home stay.

Long-term care assessments are also completed for those applying for a personal care home Social Security supplement, and for other DPW programs, such as Waiver Services. Facilities and individuals may also request a level of care determination. Casemanagment help older adults to assess their abilities and needs, explore options, develop plans to best utilize their abilities, and arrange for informal and formal supports (in-home services) to meet their needs. With an older adult looking for placement, they can assist older adults who need to enter a nursing home or personal care facility with determining the type of residential placement that will best meet their needs, and with securing such placement. They can also assist those in nursing facilities with returning home with supportive services.

Offers a geriatric care manager works with seniors and their families to guide them through the maze of resources and coordinate all aspects of care.

Senior Assessment Program at the Geriatric Center provides comprehensive evaluations performed by a geriatrician, a psychiatrist, and a licensed clinical social worker to lay the foundation for quality ongoing care. This care assists an older individual with the ability to enjoy good health, improve overall quality of life, reduce the need for hospitalization, plan for long-term care, as well as live independently for as long as possible.

Provides comprehensive in-person assessment for older adults to determine the level of care they need and the benefits and services for which they are eligible.
