Provides case management services and relocation assistance to make permanent, affordable housing a viable option for homeless individuals and families living with HIV/AIDS. Pathways is a 24-month transitional housing for single adults. Choice I & Choice II offer permanent housing for single adults and families throughout Allegheny County. Case managers will provide referrals for primary medical care, nursing care, drug, alcohol, and mental health treatment, nutrition counseling and money management education.

Eligibility: Homeless individuals and families currently living with HIV/AIDS. The individual or family head must prove motivation in working on personal goals and issues related to homelessness.
