Serves as the central hub for intake and registration for supports coordination for eligible individuals.

Eligibility: Allegheny County residents with Intellectual Disability (ID), Autism, Developmental Disability (DD) prior to age 9, and/or Medically Complex Condition (MCC) diagnosed prior to age 21. Must also meet additional Pennsylvania Office of Developmental Programs (ODP) criteria.
Hours: Monday through Friday, 8:30am to 4:00pm, excluding holidays.

Fee Amounts: Referral services provided free of charge.
Support Services are waiver-funded.
Intake Procedure: Call the office to schedule or complete the initial questionnaire.

If initial screening deems individual likely eligible, staff will guide the individual (or parent/guardian) through the process of registration and selection of a Supports Coordination Organization (SCO).
Intake Requirements: An over-the-phone questionnaire is required to begin the registration process.

Helpful Tips: A Supports Coordinator from the selected SCO will assist in developing a plan for supports and services.
