Indiana - Family Hospice and Palliative Care
Provides relief and comfort for the terminally ill to remain in their own home. Services can include: management of pain and symptoms, provides needed medications, equipment and supplies, volunteer support, physical, speech and occupational therapy.

Palliative Care uses symptom management, pain management and comfort measures for relief for those with chronic pain or illness.

Eligibility: Hospice eligibility: A patient diagnosis of a terminal disease with life expectancy of six months or less; a desire by the patient and family for care that is aimed at providing comfort and quality of life in the home; and the availability of the attending physician.
Hours: Monday through Sunday, 7:30am to 5:00pm; 24 Hour on-call, 7 days per week
