Intake Procedure: Clients can call or walk in to the office to complete an intake form. Necessary information includes name, date of birth, address, Social Security number, insurance coverage, problems/issues, etc.
Intake Requirements: Completion of an intake form. Once the intake is completed and insurance is verified or benevolent care is approved, the scheduling coordinator will contact the client to schedule the initial appointment.
