Supports Coordination is the connecting bridge between the person with an intellectual disability/autism and the services and supports they need. Individuals with disabilities are assigned to a Supports Coordinator who locates, coordinates, and monitors services and supports for people with intellectual disabilities and autism.

Fee Amounts: Accepts Medicaid Intellectual Disability (ID) Waiver
Intake Procedure: Registration required. Visit the website to find phone numbers your county’s intellectual disabilities office and request an intake.
Intake Requirements: Must have IQ under 70 as determined by a licensed psychologist prior to 22nd birthday.
Geographical Area: Berks, Bucks, Chester, Delaware, Montgomery, and Philadelphia counties
Helpful Tips: If you qualify, the county provides a list of willing and qualified Supports Coordination Organizations (SCOs)that serve your county. Choose an SCO and inform the county of your choice. Once you are registered with that SCO, a Supports Coordinator will contact you to schedule a meeting to discuss your needs and goals.
